DMC Support provides independent support coordination and specialist support coordination for plan and self-managed NDIS participants. We have extensive experience in working with parents during pregnancy, birth and early parenting. Our knowledge of services to support you during your journey will provide you with support that is specifically tailored to meet your needs and NDIS Plan goals.

How we can help you

Our focus is on helping you develop the skills and supports you need to succeed as a parent. We do this through:

Assisting you access the NDIS if you don’t have a current NDIS Plan
Understand the NDIS and your plan
Linking you with disability-specific services & supports
Assisting you access community and relevant mainstream services
Coordinating you NDIS funded services and navigating Service Agreements
Help you prepare for your NDIS Plan Review
Liaise with the NDIA when needed

What you can expect from us

Regular and timely communication

We communicate regularly with you using your preferred method (SMS, phone, email).
We provide regular updates on the coordination of our supports for you.
We respond to your communication within 48 hours as a minimum.

We are experts in the field of navigating maternity care for people with a disability.
We provide a range of free online resources in association with disAbility Maternity Care
